This service is designed for participants who want support with planning, decision-making, identity development, routines, and capacity building, especially during periods of change. Life transitions can include changes in living arrangements, relationships, work or study goals, mental health stability, routines, community participation, or independence skills. We support participants to reduce overwhelm, build structure, and strengthen confidence through calm, relationship-based guidance.
